Application Components
The following is a brief summary of the information that is required to complete each section. Review instructions thoroughly as you are responsible for submitting correct and complete information. Instructions can be found in the help windows provided in most sections of the online application.
    
Title headers    The headers at the top of the screens contain three buttons – “Main” (includes information about the application process), “Submit” (allows you to review the information that you have entered, notes missing information and then is used to submit the completed application) and “Logout” (use anytime you wish to logout from your application). Note: You may use the banner and/or the subheadings at the top of each screen to retrace your steps.

Personal Information    This section collects your identifying information which is fundamental to the application process. You must provide your full legal name, date of birth, citizenship, and language information. Permanent residents will be required to provide their date of landing.

Address Information    Home, mailing, email, and rural origins information are gathered in this section.

Questions    The medical schools require answers to several questions concerning your postsecondary academic experience.

Choices    Select the medical schools to which you are applying.
Sketch    This section collects detailed biographical information about you and your activities since age 16, organized into seven different categories.

School Submissions    Some of the schools require you to answer additional questions or to submit an essay. All of these requirements are found within the OMSAS online application. It is important to note that there are limits to the length of your answers.

Referees    You will be required to ask three individuals to provide a reference on your behalf using the Confidential Assessment Forms (aka Referee Forms). You must provide their names and addresses.

MCAT    Provide the information required for the collection of your MCAT scores in this section.

Postsecondary Education    You will be required to enter detailed information about the universities and/or colleges that you have attended or are currently attending. You will need a copy of your transcripts so that you will be able to accurately enter details for each undergraduate program and course, including grades.

Transcripts    If you have attended universities in Ontario, you must request your transcripts by completing this section. The OUAC has been processing transcripts “electronically” since 1997 with the Ontario universities.
